- Print a Bloodborne Pathogen Form - Do not send on to HR, file with Department's Exposure Control Plan.

• Other Forms - some additional forms should be completed by employee via Self Service/FUS Web Applications under the "My Information" section. Once completed, information will be updated immediately in the system.
- W-4 Forms, both Federal and State. If not received, the defaul status is single, 0 allowances (the highest withholding rate)
- Direct Deposit (if employee elects to have checks deposited directly into a bank account)
- Address Form
- Emergency Contact information
- Employee Education Degree Information
Other attachments that are required for specific forms, e.g., CV for Faculty Appointments, or any forms required for Affirmative Action will be routed along with the form electronically in workflow. Electronic routing of attachments.
