Request for late/retroactive withdrawal of entire registration
This form should be used by any College of Liberal Arts and Sciences (CLAS) student to request collegiate authorization to withdraw all courses from the current or previous semester/session after the published deadline. This form should be used only by undergraduate students enrolled in CLAS during the semester in question. Reinstatement assessment
This form should be used by former College of Liberal Arts and Sciences (CLAS) students who were dismissed from the college and have already scheduled a reinstatement interview with CLAS Undergraduate Programs. If you have not yet scheduled a reinstatement interview, do not fill out this form, instead contact clas-undergrad@uiowa.edu or 319-335-2633 for assistance. This form should be used by current or former College of Liberal Arts and Sciences (CLAS) students who have returned to the University of Iowa to complete their first undergraduate degree after an enrollment gap of at least four years, and who want to have one for more CLAS semesters/sessions removed from GPA calculations.
